နဲ့ဆိုင္တာေလးေတြေရးပါဦးမည္

PVCs

ဒီေန့ အသက္ ၂၁ နွစ္ female patient
တေယာက္က palpitation နဲ့ လာျပ ေတာ့ ECG
မွာ PVCs ေတြ့ပါတယ္ heart မွာေတာ့ skipped beat ႀကားရပါတယ္ PVCs က အေတာ္ common
ျဖစ္ပါတယ္ မႀကာခဏ ေတြ့ရလို ့ medscape က
တဆင့္ သူ့ guideline ေလး ဖတ္မိသေလာက္ေဝ
မ်ွတာပါ

PVCs က premature ventricular contraction
ပါ နွလံုးမွာ နွလံုးခုန္ေအာင္ စေပးတဲ့ pacemaker
က SA node ပါ PVCs မွာေတာ့ နွလံုးခုန္တာကို
AV node ေအာက္က ပါကင္ဂ်ီ ကစပါတယ္ ဒါေႀကာင့္ ventricle ေတြက atrium အရင္ ခုန္ပါ
တယ္ ဒါေႀကာင့္ cardiac output က inefficient
ပါ ventricle ကလာ တဲ့ အတြက္ QRS complex
က wide and bizarre ပါ p wave မရွိဘူး
PVCs ေတြႀကားက normal QRS အေရ   အတြက္
1,2,3 ေပၚ မူတည္ျပီး bigeminy, trigeminy,
quadrigeminy ဆိုျပီးေခၚပါတယ္ geminy က
twin ကို ေခၚတာပါ
normal complex တခု နဲ့ premature complex  တခု တြဲရင္ နွမႊာပူး bigeminy ေပါ့

normal ၂ ခု PVCs တခု ဆို၃ မႊာပူး trigeminy ေပါ့

normal ၃ ခု သူတခု ဆို၄မႊာ ပူး quadrigeminy
ေပါ့ ေအာက္မွာ ဒု တ စတုတၳပံုေတြက အာ့ ပံု
ေတြပါ

PVCs ျဖစ္ေစတဲ့ mechanism က ၃ မ်ိဳးပါ

enhanced automaticity
reentry နဲ့
triggered activity ေႀကာင့္ပါ

automaticity ကေတာ့ ectopic foci ေတြေႀကာင့္
ပါhypoxia catecholamine မ်ားရင္ electrolyte
imbalance ေတြမွာ ျဖစ္ပါတယ္ calcium မ်ားရင္
potassium , magnesium နဲရင္ျဖစ္ပါတယ္
coffee မေသာက္သင့္ပါဘူး

reentry ကေတာ့ refractory မတူတဲ့ tissue
၂မ်ိဳး  close loop ျဖစ္ေနတဲ့အခါ impulse က

underlying heart disease ရွိရင္ေတာ့ mortality တက္လာပါျပီ ကုဖို့လိုပါတယ္

hypoxia
oxygen ေပးပါ ABC ႀကည့္ပါ

electrolyte imbalance
ဒီမွာ pot နဲ့ magnesium ကိုေပးပါ

drug toxicity ေတြကိုtreat လုပ္ရပါမယ္
ဒီမွာေတာ့ trycyclic ဆို bicurf
digoxin ဆို Fab antibody
aminophylline ဆို hemodialysis လုပ္ပါတဲ့

infarct ရွိရင္ေတာ့ hemodynamic ကိုႀကည့္
